1. Acceptable proof of having received 的 bacterial meningitis vaccination

• A form with 的 signature or stamp of a 医生, 被任命者, or public health personnel with 的 agency name and address; which documents when (month, 日期, four-digit year) 的 vaccination or booster was administered.
• An official immunization record generated from a 状态 or local health authority.
• An official record received from school officials, including a record from ano的r 状态.

2. Acceptable documentation for medical exemption

• A certificate signed by a 医生 who is duly registered and licensed to practice medicine in 的 United States.  其中指出, 根据医生的意见, 的 vaccination required would be injurious to 的 health and well-being of 的 student.
• A letter on 的 医生’s letterhead with 的 above 状态ment that includes 的 医生's signature and 日期.
• Prescription notes and letters over 2 weeks old will not be accepted.

3. Acceptable documentation for taking 只有 online courses

• Complete 的 细菌性脑膜炎 Online Waiver in ace
• Select 的 college you wish to attend.

4. Acceptable documentation for conscientious objection exemption

• 的 Department of State Health Services community college conscientious objection form.
• Complete 的 requested information.  打印证书.  You must use 的 form’s print button.  的 correct exemption certificate will have a number in 的 upper right-hand corner. 
• 的 Department of State Health Services Affidavit for Exemption from Immunization.  This form must be notarized and mailed to Magnus Health, copies are not accepted.

1. To submit vaccination records, or medical exemption

• 登录ace. 转到Student选项卡.
  • Click 的 HOLDS link located under Academic Profile.
  • Look for 的 bacterial meningitis hold. You will not see 的 hold unless 的 law applies to you.
  • A “BM” will show you meet 的 criteria and must submit 的 documentation before 的 hold is removed to allow for registration.

• Your college enrollment team will review your documents and contact you if your documentation is rejected. Your hold will be removed within 2到3 business days after approval.

2. To submit 的 conscientious objection exemption

• 登录ace.  转到Student选项卡.  Click 的 HOLDS link located under Academic Profile. Look for 的 bacterial meningitis hold.  You will not see 的 hold unless 的 law applies to you. A “BM” will show you meet 的 criteria and must submit 的 documentation before 的 hold is removed to allow for registration.

• If you are submitting 的 community college conscientious objection form (http://corequestjc.dshs.texas.gov/) you will upload in 的 same ace portal.
  
  
• Your college enrollment team will review your documents and contact you if your documentation is rejected. Your hold will be removed within 2到3 business days after approval.
  
  

3. To submit 的 online courses 只有 waiver

• 在“开始这里”选项卡下, click 提交 细菌性脑膜炎 Forms Here link in 的 细菌性脑膜炎 box.  You will be redirected to 的 ace document upload portal. Complete 的 process as directed. 
  
  
• Your hold will be removed once 的 online course waiver affidavit is submitted.  No forms are required for this waiver.  This hold will be waived for three weeks to allow you to register; if you are not able to register within three weeks, you will need to resubmit 的 online course waiver. 
  
  

学生 receiving postponement due to illness must still meet 的 deadline of submitting 细菌性脑膜炎 vaccination documentation no later than 10 days prior to 的 start of 的 semester.

Where to get 的 vaccination

Documents will no longer be submitted to any of 的 阿拉莫大学.

的 meningitis vaccine recommended by 的 Centers for Disease Control and Prevention (CDC) are MCV4 and MPSV4 which offer 的 protection for meningitis groups A, C, Y和W-135. MCV4和MPSV4是 只有 目前接受的疫苗. Please visit 的 CDC website for more information.

的 Texas Department of State Health Services and 的 American 大学 Health Association Immunization Guidelines agree with 的 CDC recommendation.

 Active duty military students will need to show proof of having 的 bacterial meningitis vaccination.  的 Higher Education Coordinating Board regulations pertaining to meningitis are in 19 TAC 21.613 (vaccination requirements) and 21.614 (exceptions), which is under Title 19, Education, Chapter 21, Student Services.  21年的例外.614 do not include active duty military.

·      的 waivers for medical necessity, conscience exemption and taking online courses 只有 do pertain.

 的 bacterial meningitis vaccination expires five years after 的 日期 it is received. 的 vaccination must be within 的 five years on 的 first day of classes for 的 semester starting.

的 vaccine must be in 的 body a minimum of 10 days before starting classes.

•  Newly admitted students should obtain 的 required vaccination and documentation upon submitting your application*
• Returning students should obtain 的 required vaccination or booster and submit documentation before 的y will be able to register for 的 semester.

*Proof of vaccination is not required to be considered for admission.

的 most common reasons include:

• Record is not legible – 的 copy is not clear or important information is cut off.
• Documentation is not recognized – many doctors will use 的ir own form of abbreviation or documentation that cannot be deciphered.
• Incomplete 日期s – 日期s must include 的 day, month and year.
• Invalid vaccine – documentation submitted does not include 的 specific type of bacterial meningitis vaccine or is for an unacceptable vaccine or 的 vaccination is too old.

You will not be able to register for classes until 的 proper documentation is received.

的 bacterial meningitis vaccine is available at most doctors’ offices and private clinics, 许多大型连锁药店, and some minor emergency centers or medi-clinics.  It is best to call to make an appointment at most of 的se sites.

学生 with medical insurance, Medicaid or CHIP: 

的 first thing is to make sure your insurance covers 的 immunization.  的n contact your established medical provider to see if 的y offer 的 vaccine.  If you choose to go to a different facility please call to be sure 的y accept your insurance.

学生 without medical insurance or whose insurance does not cover 的 bacterial meningitis vaccine: 

·      If 的 student is 18 or younger 的y may qualify for 的 Texas Vaccines for Children Program.  的 United Way helpline, 211, will help find providers who participate in this program.

·      Many health departments offer 的 meningitis vaccine as part of children and adult immunizations programs.  You should call to confirm that 的y offer 的 meningitis vaccine for 的 student’s age and circumstances.  A list of “full service” city and county health departments in Texas can be found at http://www.dshs.state.tx.我们/District域/铲运机.shtm  

·在圣安东尼奥, 的 University Health System will provide certain vaccinations at 的ir clinics.  Be sure to call to confirm 的 clinic you choose offers 的 bacterial meningitis vaccine.  You can get 的 list of clinics at http://www.universityhealthsystem.com look for 的 “Clinic 信息rmation” section.

Will a vaccination from my home country fulfill 的 阿拉莫大学 requirements? 

A vaccination record from a country outside 的 U.S. 能满足要求, if 的 vaccine given meets 的 CDC requirements and if 的 vaccination record is in English.  If 的 vaccination record is in a language o的r than English, an English translation of 的 document will be required.

How far in advance should I plan to arrive in 的 U.S. in order to receive 的 vaccination? 

If you plan to receive 的 vaccine in 的 U.S. you should arrive no later than 3 weeks prior to 的 first day of classes.  This will give you enough time for 的 10 day waiting period and time for your documents to be processed.  You must receive 的 vaccination at least 10 days before 的 first day of classes for 的 semester.

Can I present medical documentation from my country in order to get an exemption due to medical reasons?  

Unfortunately, 的 law requires that you visit a doctor in 的 U.S. to be able to get an exemption for medical reasons.  We suggest that you bring an English translation of your records with you, so that you can present 的m to a U.S. 医生.  Be sure your travel arrangements will give you enough time to see a 医生 in 的 U.S. and submit your documentation to 的 college.

How can I get an exemption for reasons of conscience?
